Grant applications are reviewed as soon as they are received. Grants can be awarded to assist Alzheimer’s or dementia patients and their families with expenses related to Alzheimer’s or dementia treatment and care. Requests are accepted only by those applicants who carry an active diagnosis of Alzheimer’s or dementia. Grant funds arepaid directly to a third party, such as a respite care facility or company, medical provider, pharmacy, landlord, or utility company.
Grant Eligibility
- An Alzheimer’s or dementia diagnosis to the principal working member or caregiver of the family that prevents such members from providing economic support.
- An Alzheimer’s or dementia diagnosis to members of the family in a situation of economic hardship due to expenses that are not covered by insurance.
- The Alzheimer’s or dementia patient must live within Adams County Nebraska.
Application Information of Individuals/Families
In addition to the grant application, individuals/families applying for assistance are asked to submit a letter in which includes the following:
- Name of patient and contact information
- A description of the individual's or family’s needs being addressed.
- Description of how funds will be used (third party payee)
- Brief family budget, indicating financial resources and current expenses and financial obligations.

Every effort will be made to review requests as quickly as possible.
